The Lamb's Head as a Model for Surgical Skills Development in Endonasal Surgery.

Objectives To test the viability of a lamb's head model for sinonasal surgery training in Brazil by comparing performance of endoscopic procedures in the septum and inferior turbinates. Design Prospective study. Settings Universidade Federal do Rio Grande do Sul (2015). Participants Ten physicians performed septoplasty, inferior turbinoplasty, and middle turbinectomy on 30 lamb's heads. Main Outcome Measures Degree of difficulty and similarity, opinion of tissue color and texture, and degree of satisfaction. Results Participants completed a questionnaire at the end of each procedure and a satisfaction survey at the end of the project. Significant between-group differences were found for the perceived degree of difficulty in performing left inferior turbinoplasty, right inferior turbinoplasty, and septoplasty. Final satisfaction with training was scored 9 out of 10, and all participants reported that they would like to receive such training again. Conclusion The tested model is feasible due to its low cost, its easy availability, and its similarity to human anatomy in terms of dimensions of relevant structures and tissue consistency. It allows effective training in instrument handling and acquisition of the surgical skills related to two-dimensional vision needed for endoscopy. However, it should not be considered a substitute for training of specific surgical techniques.

Doença de Behçet e a visão do otorrinolaringologista / Behçet’s disease as viewed by the otorhinolaryngologist

A doença de Behçet é um distúrbio inflamatório autoimune, que acomete pequenos vasos sanguíneos. O diagnóstico da síndrome é clínico e necessita a presença de úlceras orais recorrentes, úlceras genitais e desordens oculares. No entanto, diversos sistemas podem ser afetados. Na avaliação otorrinolaringológica, as lesões orais são os sintomas mais frequentes da doença. O paciente do caso desenvolveu odinofagia no pósoperatório de cirurgia para correção de aneurisma. Ao exame, foram identificadas úlceras orais e lesão ulcerada na base peniana. Foi aventada hipótese diagnóstica de doença de Behçet e iniciado terapia com corticoide e colchicina. Embora a doença de Behçet não seja frequente na prática diária, os profissionais médicos devem estar atentos a sua apresentação

Behçet’s disease is an inflammatory autoimmune disorder that affects small blood vessels. The diagnosis of the syndrome is clinical and requires the presence of recurrent oral ulcers, genital ulcers, and ocular disorders. However, several systems may be affected. On otorhinolaryngological evaluation oral lesions are the most frequent symptoms of the disease. The patient described here developed postoperative odynophagia after surgery for aneurysm. On examination oral ulcers and ulcerated lesion on the penile base were found. A diagnosis of Behcet’s disease was hypothesized and therapy with corticosteroids and colchicine was initiated. Although Behcet’s disease is rare in everyday practice, medical professionals should be aware of its presentation

A Vertigem Posicional Paroxística Benigna (VPPB) é a mais comum das vertigens periféricas. É geralmente descrita como vertigem transitória provocada por movimento rápido da cabeça associada a nistagmo torsional. A teoria mais aceita para a fisiopatologia é a presença de canaliculolitíase nos canais semicirculares posteriores. O método diagnóstico é a história clínica e o nistagmo presente no teste de Dix-Hallpike. As manobras de Semont e de Epley são as mais utilizadas para terapêutica.
